Support Coordination

Support Coordination is a service designed to help individuals with disabilities navigate the NDIS system and make the most of their funding. Support coordinators assist participants in understanding their plans, connecting with service providers, and coordinating various supports and services to achieve their goals.

At Disability Destinations, we offer Support Coordination services, as a separate entity to our other services. This means, we do not refer services back to ourselves. This keeps the conflict of interest concerns in check for everyone.

The main roles of support coordinators include:

1. Plan Implementation: Helping participants understand and implement their NDIS plan.
2. Service Coordination: Connecting participants with appropriate service providers and supports.
3. Capacity Building: Empowering participants to manage their own supports over time.
4. Problem Solving: Assisting in addressing any issues or barriers that arise in accessing services.
